Teacher Reimbursement Request Form

WIS PTO Supports Teachers

Use this form to submit receipts for reimbursement through the WIS PTO Teacher Discretionary Fund. Each teacher is eligible for up to $150 per school year.

Instructions

Complete all required fields.
In the Description section, provide a brief explanation of the purchase (for example: pencils, markers, classroom books, or science supplies).
Upload up to five receipts or supporting documents (photos, scans, or PDFs are accepted).
Choose whether you would like your reimbursement check delivered to your school mailbox or mailed to your home.
The WIS PTO Treasurer will email you when your check is ready or has been mailed.
You will receive an email confirmation with a copy of your submission for your records.
